1 min read

Daily Pet Quality of Life Log Track your aging pet's daily comfort to guide end-of-life decision making with your veterinarian.

Pet Name: ___________________________ Species/Breed: ___________________________ Veterinarian: _________________________ Target Good Days %: ________________________

How to Score (0 to 2 for each category)

0 = Poor / Struggling: Severe pain, refuses food, cannot move without help, or completely withdrawn.

1 = Moderate / Managed: Showing signs of aging or discomfort, but responds to medication, eats some food, or still interacts.

2 = Good / Normal: Comfortable, happy, eating well, moving easily, and enjoying life.

Rule of thumb: If the daily total score is under 5, or if 'Bad Days' consistently outnumber 'Good Days', consult your vet.
